This form must be completed for each traveler.  This form will ask for basic personal information and emergency contacts.   If you do not want to purchase the travel insurance then this form can be be used to waiver your travel insurance as well.

This form is used to apply for a U.S. Passport for the first time.  A difference form is required if you are applying for renewal of an existing passport

This form is required by certain cruise lines if you are traveling with a minor under the age of 18.

This form is completed to authorize credit card charges to be made via phone or in person to your Travel agent. This information will be held in confidence and will not be shared with anyone who does not work for Lov2escape Travel Agency

This form provides useful information for first time cruisers.
